Admission Guidelines for Successful Candidates 2025–2026
Step 1: Initial Fee Deposit (UHS)
- Deposit Rs. 600,000/- in the designated UHS account within the timeframe prescribed by UHS.
- Failure to deposit the fee within the stipulated time will result in cancellation of the admission offer, and the seat will be offered to the next candidate on merit.
Step 2: Joining Report & Consent form
- Fill in joining report or upgrade option on UHS portal.
Step 3: FMHCMD Application Form (Stay Option Candidates Only)
- Complete the FMH College of Medicine & Dentistry Application Form at: https://fmhc-oaa.almusnet.com/#!/auth/signin
Step 4: Submission of Documents (Stay Option Candidates Only)
Candidates must report to college along with following documents
· Matric / Equivalent Certificate – Original + 5 attested copies (both sides)
- Sc. (Pre-Medical) / Equivalent Certificate – Original + 5 attested copies (both sides)
- MDCAT /Equivalent Result Card – Original + 5 attested copies
- Printed copies of both UHS and FMHCMD application forms
- Candidate’s CNIC / B-Form or Passport /Residence Permit in case of foreign students – Original + 5 attested copies
- Father’s CNIC – Original + 5 attested copies
- Passport-size photographs – 10 copies
- Domicile – Original + 3 attested copies
- Affidavit – On two (02) stamp papers of Rs. 300 denominations (Content as shared via email)
- Declaration– On one (01) stamp paper of Rs. 300 denominations (Content as shared via email)
- UHS fee deposit receipt
- Joining Report and Consent for Stay
Step 5: College Fee Submission (Stay Option Candidates Only)
- Deposit the college fee challan at any Bank of Punjab (BOP) branch and submit the deposit receipt to the FMH College Accounts Office or sent via email to fakhar.riaz@fmhcmd.edu.pk
